Your first and last name, cell phone and any up to date contact information, your current address and your email address. Make sure that we can reach you when we contact you back.
It can be very helpful if we also have the name of the pharmacy where any medications will be sent. Include the name of the pharmacy, the address, the town, state and ZIP CODE—If we have this before we speak, things go faster and smoother when we call you back to set things up.
