If you are a new patient a please follow these easy 4 steps below to complete your registration for the program and get started.
STEP 1: Read the information on this page…two or three times
STEP 2: Fill out the patient registration form
STEP 3: Go ahead and make your first online payment and we will get started ASAP.
STEP 4: We will see you in person or via telemedicine before we send in your first prescription.
Congratulations! You are now on the way to treatment with us at SUBDOC.US and we look forward to a long and productive relationship. A member of our team will be with you shortly to schedule your first visit with one of our providers.
Important: If your schedule does not allow you to be seen during normal business hours, let us know and we will pick a more convenient time.
Also, before you do anything else, make a note of our phone number in your phone. The main number is (321) 415-7918. Doing this will make it easier to contact you after registration…you will know that it is us calling you and not someone trying to sell you an extended warranty on your car (Yes-we all get those calls). Keep an eye out for a call from that number as we will be calling and sending an invitation to your phone or email (from https://spruce.care/subdocs2you) after and only after you complete the intake below.
